Interview notes that survive a handoff

A capture sheet pattern we use so the next analyst can continue without replaying every meeting.

Notes written only for yourself vanish when you leave the project. We teach a four-block sheet: context, claims, evidence, and decisions still pending.

Claims without evidence stay labeled as claims. That single habit prevents “the business said” folklore from becoming fake certainty.

End every interview with a spoken summary of what you think you heard. Corrections in the room beat corrections three sprints later.

Share the sheet the same day. Fresh stakeholders still remember what they meant.
